New Patients

New patients should bring their completed new patient packet to their appointment. We ask that new patients arrive 30 minutes prior to their scheduled appointment time if they have completed the packet, otherwise we ask you arrive 60 minutes prior to the scheduled appointment time.

 

• All new patients should bring a valid photo ID and current insurance cards, including prescription cards, to all appointments.
• We will file with all major insurance carriers.
• Copays, deductibles and co-insurance payments are due at the time of service.
