A Checklist for your New Patient Consultation

Chronic Venous Insufficiency, commonly known as vein disease, affects 40% of the American population, but few patients seek treatment for their symptoms. If you are suffering from varicose veins, spider veins, swelling, leg pain, or skin changes of the legs, set up an appointment with Restore Vein and Wellness.

During your initial consultation, a thorough History and Physical Exam will be performed by your provider. This will be followed by a Venous Ultrasound of the lower extremities and will aid our provider in diagnosing your problems. Our team will review the test results and develop a personalized treatment plan.

In preparation for your appointment, drink plenty of water, hold your diuretic(“water pills”) medications 24 hours prior to visit, and be active the day of the visit.

Documents needed for your visit include:

• Photo ID (Driver’s License, State ID, Military ID)
• Current Insurance Card(s)
• Referral from your Primary Care Physician (if required by your Insurance)
